2/18/2024 EnchantaVaughn Approved QMHPA arrived to the home of Kaila. QMHPA held an individual session with Kaila. Upon arrival at Kailaâs home, Kaila appeared withdrawn, evident by her facial expression and body language. QMHPA discussed with Kaila that the purpose of todayâs session was to focus on Kailaâs mental health. Kaila did not present any symptoms of S/I or H/I at the time of the session. QMHPA began session by processing with Kaila her current emotional state. QMHPA reviewed with Kaila triggers to her depressive states and her interactions with her friends and family members. QMHPA engaged Kaila throughout the session through the use of active listening, asking open-ended questions and providing empathetic responses. QMHPA explored positive coping skills and reviewed Kailaâs medication management. QMHPA explored with Kaila ways to improve her mental state and encouraged Kaila to utilize conflict resolution along with problem solving skills to address her current situation. QMHPA ended session by reviewing interventions with Kaila and encouraging her to provide feedback on the session. Kaila actively participated in the session. While exploring problem solving and conflict resolution skills, Kaila stated that she blocked her friendâs number because she did not want to engage in a verbal altercation with her over the phone since she was very upset. Kaila stated that she was concerned about becoming physically aggressive, so decided to refrain from any interaction with her friend at this time. Kaila shared that the positive coping skills were not helpful in this situation and that she would continue to explore alternative options for things that she needs when it comes to her being able to go to her friend (it appears this is the male friend). At the end of the session, Kaila shared that she felt people were unreliable and will be mindful of her trust towards others.
